HWLincs has completed a number of pieces of work for NHS Lincolnshire in relation to palliative care and end of life.  We also recognise with this particular cohort of patients, their family, carer and loved ones the this period in their life is tremendously fragile and we do everything we can to ensure the engagement is sensitive and in recognition of their needs.

The two current work programmes for 2022 relate to experience of hospital discharge during this part of the patient journey and post bereavement review of care with family, carers and loved ones.

The approaches we take ensure a compassionate review of individual stories which provide rich and real time insight into the services and interventions that are put in place.  This insight allows for strategic and delivery models to be developed with the people at the centre in mind.
